Privacy & Cookies
Last updated: 29 July 2026
This notice explains what information this website collects, the cookies it uses, and the choices you have. It covers our website (meridianhealth.com.ph) and our online booking pages.
In plain terms: what you tell us stays confidential. We do not sell your information and we do not share it for anyone else's advertising. A small number of trusted technical partners help us run the clinic's systems - sending your appointment reminders, keeping the booking page online - and they may only use your information to do that job for us, never for their own purposes. Personal information is encrypted. The sections below name each partner and say exactly what it receives.
It is not our full clinic privacy policy. A comprehensive privacy policy covering everything about how the clinic handles patient information is being finalised with legal review and will be published on this page. Health information you give us before or during a visit - for example in our pre-visit health forms - is always covered by a separate consent notice shown on that form itself, and is used only for your care.
Who we are
MeridianHealth Medical Clinic
196 Purok 5, Lourdes, Lubao, Pampanga
Phone: 0969 272 5514 · Open Monday to Saturday, 8:00am to 5:00pm
We process personal information in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173).
What this website collects
- What you type into our forms. When you book a visit, check an appointment, or send us a message, we collect what you enter - such as your name, contact details, and the service or message you send. We use it to arrange your visit and to reply to you. We keep website enquiries for 12 months from the date you send them, then delete them.
- How you found us.If you arrive through a partner's or creator's link, or through a link from one of our campaigns, we record the referral code or campaign tags carried by that link (see the cookies below). This tells us which of our partners or campaigns brought you here, so we can credit them fairly. It says nothing about your health.
- Your language preference, if you use the English/Taglish toggle.
We do not sell your information. The referral code is read only by our own website - it is never sent to advertising platforms. We never send your name, contact details, or the specific treatment you booked to advertising platforms; at most, advertising measurement (only if you accept it - see below) receives a broad category such as "massage/wellness" or "medical".
Cookies we use
We also keep the referral and campaign information for your current visit only in your browser's session storage (mhmc_attrib_s). It is deleted automatically when you close the tab, and is used only to fill in hidden "how did you find us" fields if you submit one of our forms during that visit.
Text messages (SMS)
If you give us a mobile number, we may send you appointment-related SMS text messages: booking confirmations, appointment reminders, and one-time verification codes. We do not send marketing texts.
These texts are delivered through Semaphore, an SMS provider based in the Philippines, which processes your mobile number and message content solely to deliver the text.
Booking confirmations and one-time codes are sent because you just took that action yourself (booking, or asking for a code) - these always go through. Appointment reminders are sent only if you have consented, and every reminder text includes a link to stop them at any time; you can also ask the front desk to stop them, or to turn them back on, at any time.
We keep your consent/opt-out record and the phone number itself for as long as we keep the rest of your record under our general retention policy (10 years after your last visit, or, if you were under 18 at your last visit, until you would turn 28) - see "Your rights" below for how to ask us about or correct this. We do not keep a separate log of what each individual text said; Semaphore may retain its own short-term delivery records under its own privacy policy.
Website measurement and advertising
If - and only if - you tap Accept on our cookie banner, we load Google Tag Manager. Through it we run Google Analytics, which counts visitors and shows us which pages lead to a booking, and advertising measurement that tells us whether our ads work (for example, on Google or Facebook/Instagram). These tools may set their own cookies and receive information about your visit, such as the pages you viewed and whether a booking request was made - never your name, contact details, or the specific treatment, and never the contents of any health form. They handle that data under their own privacy policies (Google, Meta).
One thing we want to be plain about: our page addresses are named after what is on them, so an address like /treatments/massage shows which treatment page a visit looked at. If you accept, Google Analytics receives those addresses. It does not receive your name, and we never connect them to you or to your clinic records - but if you would rather Google not see which pages you read, tap Decline.
If you tap Decline, or simply never answer the banner, these tools are not loaded at all.
Your choices
- Use the cookie banner's Accept or Decline buttons when they appear.
- Use the button below to clear the choices and cookies described on this page. If advertising measurement is running, the banner will ask you again on your next page.
- You can also block or delete cookies in your browser's settings at any time.
Your rights under the Data Privacy Act
Under the Data Privacy Act of 2012 you have the right to be informed about, to access, to correct, and to object to the processing of your personal information, and in proper cases to have it erased or blocked. To exercise any of these rights, contact our Data Protection Officer at [email protected], call us at 0969 272 5514, or visit our front desk at 196 Purok 5, Lourdes, Lubao, Pampanga. You may also raise concerns with the National Privacy Commission (privacy.gov.ph).
Updates to this notice
We will update this page when our practices change, and we will replace it with the clinic's full privacy policy once legal review is complete. The "last updated" date at the top always tells you when this page last changed.
